REGISTRATION FOR GRADES 1-5

Students New to Fulton County:

Welcome to Fulton County! Since we cannot immediately verify your child's enrollment in other school systems, you must provide the following documentation when you register. (If you are from another Georgia school, we need only proof of residency and the withdrawal form from your other Georgia school. We can send for your records and get everything else.)

Birth Certificate: A certified copy of a student's birth certificate or a passport must be presented for their record. Please bring a copy when you register, so we will have this important document. A birth certificate is REQUIRED before we can enroll a Kindergarten or first grade student.

Immunizations Record: The immunization record for a student is required at registration. All immunization records must be transferred to Georgia Form 3231 and this can be done at any health department or doctor's office.
Hearing, vision, dental: The hearing, vision, and dental certificate for a student must also be transferred onto Georgia Form 3300. Records from other states can be transferred at any health department or doctor's office.
School Records: It is helpful for class placement if you can bring a copy of your last report card. It is not required. We will request records from your previous school, but there are sometimes long delays in us getting those records. Please bring the school's address if you have it.

Social Security requirements: Social Security numbers are requested at the time of enrollment, however no student is denied enrollment for declining to provide their Social Security number. The parent or guardian must sign a written waiver if they choose not to provide their child's Social Security number.

Students Transferring from Another Fulton County Public School:
Since you have already attended a Fulton county school, the only thing we must have is proof of residency. It is also helpful to bring your last report card from your previous school. Details of other important documents are below.
A student's birth certificate, immunization record, and hearing, vision, and dental form should already be in their permanent record and we will receive it from the other school. Records from other Fulton schools usually come very quickly. It is fine to bring copies when you come to register, but we do not have to have them. If any documentation is missing when we receive their record, you would have to provide it for them to stay on roll.
