GENERAL INFORMATION Students will be sent to the clinic for illness, accidents or
injuries. A parent will be notified in the event of vomiting,
fever or diarrhea during school hours.
MEDICINE
The medical release form is required for all medications including
the over-the-counter medications. It must be signed by a
parent or legal guardian. If the medication is a prescription,
it also requires a doctor's signature.
ALLERGIES & MEDICAL CONDITIONS
It is imperative that parents/guardians keep the school informed
about any allergies or medical conditions.
CHANGES IN EMERGENCY CONTACTS
For your child's safety, please notify the front office and clinic
of any changes in emergency contact information.
