Summer Registration for NEW Students Grades K thru 5

Mondays and Wednesdays 8am-11:30am

Thursdays 1pm-3pm

Closed for Lunch 12pm-1pm

When registering your child at our school, please bring the following items:

• Current Immunization Certificate of child

• Valid Ear, Eye, and Dental Exam of child

• Birth Certificate of child

• Valid Identification of parent

• Social Security Card

• Affidavit of Residency (can be picked up in office)

• Legal Guardianship Paperwork (if applicable)

• 2 Proofs of Residency (Gas, Water, or Electric Bill ~ Lease ~ Mortgage Statement ~ Bank Statement)

• Withdrawal Paperwork from Previous School

  A notary is on staff to notarize Affidavits free of charge.
