During the last school year, our clinic saw an average of 60 children per day for
prescription medication and other medical complaints. The clinic is
busy but runs efficiently due to the help of our students, staff, and the continued communication from parents and guardians. We need your help to make your child's visits
to the clinic safe and organized. The clinic is staffed Monday
through Friday from 8:00 AM to 4 PM to meet your
child's medical needs. Please feel free to visit us or call us

All prescription medications require you to complete a
North
Fulton County Medication Authorization Form. A doctor's
signature is required on this form for all prescription
medications, such as inhalers, antibiotics, and epi-pens. All
prescription medications must be in their original containers with
the pharmacy labels intact. It is your responsibility to continue
supplying the clinic with your child's prescription medications.
We will send the original container home with your child 24 hours
prior to the medication running out.
Non-prescription medications also require you to complete an
authorization form, but no physician's signature is required, only
that of a parent or guardian. Examples of
non-prescription medications include vitamins, topical creams, cough drops, cough syrup, Tums,
Benadryl,
Pepto-Bismol, Tylenol, Advil, Midol, and flu or cold
medicines. All non-prescription medications must be in their
original containers with the child's name written on the container
and in date through the end of the current school year.
Senate Bill 472 was passed
during the 2002 Georgia Legislative Session. This bill allows children with asthma to carry inhalers for
self-administration while at school or at a school- sponsored event. As a
result, Fulton County School System has revised the Authorization For Students To Carry A Prescription Inhaler, EpiPen,
Insulin, or other Approved Medication policy and procedure.
If you have a student who has asthma or other health conditions that
require self-administration of medication, you must obtain a form

For Students To Carry A Prescription Inhaler, EpiPen, Insulin, or other
Approved Medication. This
form, including written permission from your doctor and your signature
must be completed before the school can allow your child to carry
medication.
Hand written notes, faxes, or medication not in its original
container will not be accepted for prescription or
non-prescription medications in accordance with Fulton County
Board of Education Guidelines.
If your child has any medical condition of which you would like
us to be aware that may require an emergency medical plan, please
contact us. Examples are asthma, or allergies to foods, medications, insect
bites, epilepsy, and diabetes.
Parents, guardians, or individuals on emergency contact lists
will be notified by phone if the child sustains a significant injury on
school grounds. Please provide the school with all home, business,
and emergency phone numbers, as well as cell-phone and pager
numbers. This will ensure that we are able to contact you in a
timely and efficient manner.
