Student Information

 If your student needs any information forwarded to other schools, doctor’s offices, etc., please contact Gail Ries in the Guidance Office at 770-667-2870, extension 129.  All forms will be sent from NMS to the requesting party so be sure to include full names & addresses in your request.  If the form does not have a release of information for you to sign, please send a signed note to Ms. Ries’ attention for our records. Kindly allow two weeks for this process. Thanks!

New Address?

If you have moved and have a new address, please provide the Guidance Office with two forms of address verifications (two utility bills, lease agreement, etc.) and an affidavit of residence so your student's file can be updated.

N.M.S. COUNSELORS FOCUS ON HELPING STUDENTS 

The Northwestern guidance and counseling program is an integral part of the middle school experience.  The purpose of this communication is to inform parents of the services available to assist students with their personal and academic needs.  Guidance and counseling services provided at NMS include:

Affidavits of Residence 2008-2009

After July 1, ALL students in 6th, 7th and 8th grade will need to provide a notarized Affidavit of Residence prior to the first day of school for 08-09.  The first day of school is August 11.  

All incoming 6th graders - including students coming from Crabapple Crossing, Summit Hill, Alpharetta Elementary, Hembree Springs and all new enrollees must provide the following:  notarized affidavit of residence AND two proofs of residence by August 11.

All 7th and 8th graders returning to NMS will only need to provide the notarized affidavit of residence by August 11.
