If you need to keep medications at the clinic; prescription or non prescription (Tylenol etc..) please make sure to fill out an Authorization to Give Medication at School Form (if prescription,  this needs to be signed by a doctor).  We are not permitted to give any medications that are not brought in by the parent in its original container.

If you need this form and can’t drop by the school, please click on the links on this site to help you get what you need quickly.  You may also go to the Student  Health Services website: www.fultonschools.org/dept/healthservices .  These forms are good for one school year.

Students who are sick, contagious and have a fever equal or greater than 100.4,  must not be sent to school, and be fever free without Tylenol/Motrin for 24 hours before returning. A note from the doctor may be requested by the school in order for your child to return to school.   By working together, we can strive to ensure the health and well being of every student.
