1.          Official State Birth Certificate (Pre-K)

2.          Georgia State Immunization (Form 3231)

3.          Georgia State Hearing, Vision & Dental Screening (Form 3300)

4.          Child’s Social Security Card (Optional)

5.          2 Proof of Residency: (Pre-K)

·        Copy of home mortgage payment book

·        Copy of home contract

·        Current paycheck stub

·        Receipt to have utilities connected

·        Current bank statement

·        Current utility bill (gas, electric, water)

·        Residential lease

·        Homeowner’s/Renter’s insurance registration card
