Clinic Administration
TracyLynn Wagner is the Webb Bridge cluster nurse who serves
several schools in one geographical area. She provides
assistance to parents and students with medical conditions
or concerns (i.e. allergies, diabetes, asthma, seizures,
transplants, and newly diagnosed conditions). She works
closely with the clinic assistant, Barbara Townsend, and the
school administration to train staff to follow clinic
protocol in regards to medical conditions, medication
administration, etc. She provides education and training for
the staff. Barb Townsend may refer parents to TracyLynn to
answer questions regarding specific medical conditions and
protocols.
General Information
Clinic hours are 10:00 a.m. to 2:00 p.m. Students will be
sent to the clinic for illness, accidents, and injuries. The
clinic is staffed by a part-time clinic assistant, Barb
Townsend.
Allergies and Medical Conditions
The clinic is responsible for maintaining all student health
records. Please keep the school informed about any allergies
or medical conditions that your child has. Documentation is
entered into a private database as a clinic resource.
Medicine
A medical release form is required for all medications that
are stored and administered at school, including the
over-the-counter medications. It must be signed by the
parent or guardian. A doctor’s signature is required if it
is a prescription medication.
Changes in Emergency Contacts
Please notify the front office staff of any changes in
emergency contact information. For your child's safety,
please notify the school in writing if both parents are out
of town and someone else will be acting as a guardian in
your absence.
Missing Forms?
Authorization To Give Medicine at School: If
medication can be given at home or after school hours,
please do so. However, if medication must be given during
school hours, this form must be completed.
Authorization To Carry Medications:
Authorization For Students to Carry A Prescription Inhaler,
EpiPen, Insulin, Or Other Approved Medication (Other
approved medication shall be defined as prescribed
medication used for emergency purposes and/or medication
approved by Student Health Services in collaboration with
the student’s health care provider.)
